The Fellowship Haven Church in Woodburn, Indiana, USA was established in 1976. The congregation started from a group that withdrew from the the Ridgeview Amish Mennonite Church in 1971. Originally they affiliated with the Fair Haven congregation near Goshen, but the distance (65 miles) led to formation of the new congregation.

In 2017 the church was a member of the Beachy Amish Mennonite Fellowship and had a membership of 52. The bishop was Roman Miller, and the minister was Tim Eicher.
