The Fort Wayne Mennonite Church in Fort Wayne, Indiana, USA, began in 1960 as a church plant of the First Mennonite Church in Berne, Indiana. At first, a small group met in the Fort Wayne YMCA. The Berne church then purchased a parsonage in a developing suburb on the northeast side of Fort Wayne. The parsonage served as the congregation's meeting place for three years. When the suburb became known as Maplewood Park, the church took the name Maplewood Mennonite Church.

In 1961 the new fellowship purchased four acres of land across the street from the parsonage and built a church building. The Maplewood congregation held its first service in the new building on 23 December 1962. It soon began to host a nursery program in the new facility. The congregation enlarged the building substantially in 1974 with a new sanctuary and eight additional Sunday school rooms. The sanctuary was dedicated on 2 June 1974.

In 2022 the congregation was part of the Central District Conference of Mennonite Church USA.
