The Calvary Chapel in Louisville, Ohio, USA was established in 2006 when a group withdrew from Christian Fellowship.

In 2017 the church was a member of the Beachy Amish Mennonite Fellowship and had a membership of 30. The bishops were Sylvan Weaver and Leonard Overholt and the minister was Ronald Border, Sr.
