The Swan Lake Amish Mennonite Church in Avon, Illinois, USA was established in 2016. The congregation started as an outreach.
In 2018 the church was a member of the Midwest Beachy Amish Mennonite Church and had a membership of 33. The ministers were Marvin Ray Yoder and Stephen Byler.
